In my case it's almost twice what MultiPCB charges for S&H.

Yeah, fiscal reasons, maybe. But I recommend them as a very satisfied
customer.

Actually, I've never ordered a single board from them (maybe a single
panel once if memory serves), because the incremental cost is so low.

Indeed, their history is weird. Apart from selling to individuals, they
also offer as standard (low cost pooled) 4 layer with RF dielectric for 
the outer layers, which is something I need in another project.

Thanks for Basista, it's the first time I see blind/buried vias on an
online calculator.
