Sure there is a 640 * 200 resolution: mode 6 (CGA, 2 colors) and mode 14
(EGA and VGA, 16 colors).  But EGA has 640 * 350 with 16 colors, and VGA has
640 * 480, so we rarely see 640 * 200.  Now this is largely superseded by
SVGA.
