To answer the d & e 'what is this for?':

Dan and I added that to make dealing with low cost house like APCircits easier.

The board could be sent to different houses without having to change
the PCB file itself.
Load the drill map and regenerate the drill file and perhaps the gerbers.
Each board house had/has their own standard drill rack.

Without this you had to have different PCBs for different houses to
match their drill rack.
