The issue comes up when one doesn’t understand the extent of possible damage. I once saw a change in a shared library cause a tool to delete a common database, shared among multiple users. The tool itself was only supposed to read the database but the documentation and implementation of a flag passed to a particular function were ambiguous. After several “huh?” moments followed by restores from backup tapes, we finally figured out what was happening.
