Bread board is very likely an issue.
The output drivers switch high current low-side and high-side.
If ground supply of the driver module shifts relative to your microcontroller
then it can easily detect wrong signals.
It can also have problems on the power output, because of undervoltage
spikes triggering temporary driver shutdown, etc..
