See Eli's answers on extenders vs DPMI providers.  Nothing is needed for
windows at all, you need a DPMI provider under DOS.

You can add cwsdstub to the start of the image instead of the default stub.
This allows you to have single exectuable distributions which will run
on either windows or under pure DOS.
