I had this nasty problem too. After some tweaking I found out that Allegro
was using a 22KHz mixing rate internally, regardless of the sampling rate I
specified into the sound init functions. The only workaround was putting the
sampling rate specification also into ALLEGRO.CFG.

Now my only complaint is about the low volume I get through my SB Live
(compared to other DOS SB programs) also with the Allegro volume set to
max...
