This morning RHIDE spit out a bug report. The cause was
very simple:

In the WindowList (which opens with ALT-0) I deleted all
files listed until it said <empty> and then I pressed
enter, with this dramatic result.

I have reproduced the problem on a MS-DOS 6.22 machine.
