Your code example was worth a thousand words! I had tried to program the
8159 interrupt controller, but I didn't think I was doing it right.
Well, I did have that part right. The part I messed up on was the
interrupt number to IRQ number mapping! It's always the little things
;-)

You mentioned that your code does not do locking, so virtual memory
should be turned off. I think there might be another hazard as well. PCI
devices are allowed to share interrupts, so if the PCI device is sharing
the interrupt, then your hook_interrupt and release_interrupt will not
be safe. But, that's another topic.
