This code looks fine to me, and works perfectly on my machine! The only
problem is that you don't set the palette before calling the mode select
dialog, so all the colors are messed up, but that shouldn't be able to
cause a crash.

All I can suggest is that, since you say the test program works ok, you
copy the initialise code from that and try to work out in exactly what
way it differs from your own routine...
