The solution depends on whether the devices are mapped below or above
the 1MB mark.  Section 18.4 of the DJGPP FAQ list describes the
former; section 18.7 should get you started with the latter.

the latest version of the FAQ is available as v2/faq210b.zip from the
same place you get DJGPP.

There's no book that I know of.  The available sources of info are (1)
the library reference, (2) the FAQ, and (3) resources on DJ Delorie's
