None.  You usually get the byte at the end of the previous allocation you
made (always in the case of unixy sbrk).

Does not effect the use.  Does change the potential behavior, since sbrk()
allocations in the standard sbrk() need not be contiguous (or even strictly
increasing).

Patch isn't needed for r2, which is current.  Standard sbrk() does some pooling
of sbrk() requests (into 64K zones) but you can end up with one DPMI memory
block per zone if you sbrk() 64K.  Unixy sbrk() uses a single memory zone and
resizes it.  Resizing can result in relocation, however, and has some problems
on some DPMI providers.

The DPMI provider must keep track of the number of DPMI memory blocks allocated.
Our current sbrk() algorithm pools requests so nothing less than 64K is 
requested from DPMI to keep the overhead down a bit.

The fewer the better, but it's not an absolute requirement.

Because there is an address and size associated with each allocation, and
you might ask DPMI to free that memory.

If you call the free memory services in DPMI, yes.  In DJGPP this is only
done at image exit.
