99% of the time it's a bad pointer.  Under non-CWSDPMI providers I don't
know of any other cause than a bad pointer.  Under CWSDPMI, you can also
see this if you didn't lock your hardware interrupt handlers, or if you
hit the internal stack space bug (only for big programs) ;-P
