You would need to write a boot sector which could load a secondary bootstrap,
which would load the image.  The image would need to be linked with a custom
libc (which you must write) which only calls the bios (or read/writes 
ports directly - DOS won't be present).  You would need to handle your own
file I/O as writes directly to the disk - in short - basically your own
operating system.
