I've finished adding namespace std support to the standard C header
files.  Shall I post the diffs here one by one?

Three of the C95 standard headers don't require patching.  These are
assert.h, iso646.h and limits.h.  These contain only macros, except that
assert.h has a prototype for the function __dj_assert.  I'm assuming
users shouldn't call __dj_assert directly, so it doesn't need to be in
namespace std.
