No, if an application is going to access the HMA they must make sure
it's "on".  This is some nonsense about old applications using 20-bit
wrap to access the interrupt table with large segments.  In any case
the VCPI provider should have still mapped the HMA pages to point to
the interrupt table to emulate address wrap.

I don't know for sure what to do in this case to try and get the EMM
to map the pages properly for the HMA.
