I use this patch for a several months. One of projects couldn't be compiled 
without this patch :-) Mixed case directory names and rename are not 
supported. To support mixed case directories cygwin.dll have to maintain 
"current directory" itself, and use NT kernel calls instead of "official" 
win32 calls.

It's not neccessary - the patch does nothing if CYGWIN32 is not set to "  
mixed"
