ack. i found the problem in the end :) the 90% of the time it was working
was when the program was running, however the application terminated without
me realising and when the callback occurred again it generated the error
because it had exit()'d without releasing the callback :)

thanks to everyone who helped, particularly eli who deserves a medal for
his patience.
