I switched some declarations around, from: typedef HRESULT (STDAPICALLTYPE FNCOCREATEINSTANCEEX) (REFCLSID, IUnknown*, DWORD, COSERVERINFO*, DWORD, MULTI_QI*); FNCOCREATEINSTANCEEX *pfnCoCreateInstanceEx; to: HRESULT STDAPICALLTYPE (*pfnCoCreateInstanceEx)(REFCLSID, IUnknown*, DWORD,COSERVERINFO*, DWORD, MULTI_QI*); And this also works around my problem.
