Missing documentation was the major flaw at the time.

The comment about them being close to each other was never clear,
wasn't inclined to 'Read the Source Luke'.
May be related to can't over lap without controlled depth drilling:

4 layer board, single blind vias (layers 1-2 or 3-4), price = X + 1/2-2/3X.
