Not always. In much of my work I'm not allowed to use pth for any kind of mounting hole, which would include holes for screwing into things like heat sinks and packages that might be included in a footprint.

I was in the aerospace industry for years and I agree.  Mounting holes shall not be plated.  I no longer remember why.  Probably due to a failure or two traced to the pth mounting holes - A reliability issue.  Ignoring history and thinking outside the box is not always a good thing to do.
