1) Report the exact name of the video board and driver you are using so I can 
create a database with problematic drivers.
2) Try upgrading your windows drivers and any TSR you are loading that's 
related to the video board.
3) If you are loading a VESA(VBE) TSR try not loading it, or the reverse.
4) Try using pure DOS, no windows GUI, no video TSRs loaded. Does it 
happend again?
