This happens to me when I hit the Start key, then alt-tab back into Quake. Seems like Win95 doesn't handle the Start key right (surprise), and leaves the keyboard in a funny state. The only fix I have is to hit Start again and use the mouse to click somewhere on the desktop (removing the Start menu).
