I found the source of the problem with the interrupt handlers causing
a page fault.
It is with the new versions of CWSDPMI

I downloaded CWSDPMI beta5, beta6, beta7, beta8 and beta9.

beta6 and beta7 worked perfectly (well they didn't page fault) but 
beta8 and beta9 both would page fault consistently when setting up the
interrupt vectors.

Hope the Sandmann can fix the bug easily, as I would like to use the beta9
version.  It has much nicer interface for customizing the swap file.

I think there also might be a bug when the swapfile is set to "", or maybe
its just I was trying to run on a 4 Mb machine and it ran out of memory.
I'm not 100% sure.

I'll stick to beta6 or beta7 for the meantime though.
