You have to compile the program makeref.cc and run it. There is a makefile
in there (djgpp\docs\djgpp) which do the job (you need "make" for that). 
This produces libc2.tex. With emTeX you get an error when compiling the
TeX-document.:

! TeX capacity exceeded, sorry [save size=1000]

I'm successful in doing the job on a Unix-Workstaion.
