As for which termcap.h to use, Chuck Wilson is working on a new ncurses release to put the ncurses includes into /usr/include/ncurses. It the current distribution he renamed the ncurses termcap.h to termcapn.h. Funny you should mention the setdtablesize problem for bash. I ran across it today building bash also. I couldn't find a definition for setdtablesize so I just modify bash. Google returned lots of pages of configure, configure.log, etc. but no description.
